Reach out by phone or email. We'll gather basic information about the individual and confirm waiver eligibility before scheduling an intake.
We conduct a thorough intake meeting with the participant and, where applicable, family or guardian to understand their current situation and goals.
Your assigned case manager develops a Person-Centered Plan in collaboration with the participant, reflecting their unique voice, strengths, and priorities.
Regular contact, annual plan reviews, service coordination, and advocacy — we're with you for the long haul, not just at enrollment.

We are seeking a qualified CFCM case manager to join our growing team. Candidates must meet BHDDH/EOHHS qualification standards for case management, including relevant education and experience working with adults with intellectual and developmental disabilities.
Requirements: Bachelor's degree in a human services field + 1 year experience (or equivalent); knowledge of RI HCBS Waiver; WellSky experience a plus.
